Angels Care Hospice is seeking a Hospice Registered Nurse (RN) to coordinate all aspects of home care, conduct admissions in the patient’s home, and determine eligibility for services. The role requires an RN license, valid driver’s license, CPR, and prior nursing experience.

We offer a $10,000 sign-on bonus for full-time hires, plus salary, mileage, on-call pay, generous PTO, extensive training, and comprehensive health/dental/vision benefits.

Formación

• Graduate of an accredited nursing program.
• Current RN license.
• Valid driver’s license and reliable transportation.
• 1 year RN experience (2 preferred).
• BLS/CPR certification.
• Hepatitis consent/declination on file.

Responsabilidades

• Coordinate all aspects of home care for the patient.
• Interact with other medical disciplines as needed to provide care.
• Perform admission visit to the patient in the home.
• Determine eligibility for home care services.
• Develop and follow the plan of care for the patient.

Educación

Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing

Current state license as a Registered Nurse

Current state Driver’s License

One-year’s experience as a Registered Nurse, two preferred

Proof of CPR

Reliable transportation with valid auto liability insurance

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
