Job summary

Angels Care Hospice is seeking an RN Case Manager to coordinate all aspects of the patient’s home care and collaborate with the medical team. You will conduct admission visits, assess eligibility for home care services, and develop the plan of care to be followed.

Qualified candidates are graduates of an accredited nursing program, hold an active RN license, and have CPR certification. One year of RN experience is required, with two years preferred, plus a valid driver's license and reliable

Qualifications

• Current RN license in the state and valid driver's license.
• Minimum one year of RN experience; two years preferred.
• CPR certification and Hepatitis consent/declination on file.
• Reliable transportation with insurance.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
