Responsibilities of a Hospice RN / RN Case Manager:
- Report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.
- Act as the primary liaison between the physician, patient/family, and hospice team.
- Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload while building trusting and compassionate relationships.
- Empower patient’s caregivers by educating them on care for their loved ones.
- Manage and collaborate with an interdisciplinary team including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
- Evaluate patient and family needs and assist in the development and execution of the care plan for each patient.
- Maintain all assigned patient clinical records.
- Participate in quality assessment and performance improvement programs.
Requirements for Hospice Registered Nurse / RN Case Manager:
- Compassionate, kind, and empathetic nature towards patients and families.
- Comfortable providing care in the patient's home or preferred setting.
- Flexibility and composure in supporting others during crises.
- RN license in the state of employment (or eligibility to obtain).
- At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Additional experience in hospital settings such as Staff RN, Med Surg, PACU, ER, critical care, etc., is advantageous.
- Valid driver’s license and automobile insurance.
- Current CPR certification.

Salary Range: $74,000 - $84,000 per year.
