Job summary

Trinity Hospice is seeking an experienced Registered Nurse to join our care team in Denver. You will administer skilled nursing at patients\' residences, admit patients to the program, and conduct regular visits to ensure quality end-of-life care.

You\'ll coordinate with a hospice interdisciplinary team and communicate with physicians to meet patient and family needs. We value accountability, flexibility, and integrity.

Qualifications

• Current Registered Nurse license in the state where you will be working.
• One year of experience as an RN Case Manager or in a similar hospice role is preferred.
• Must have a valid driver\'s license and dependable transportation.

Responsibilities

• Administer skilled nursing in patients\' homes.
• Conduct patient admissions.
• Perform routine patient visits.
• Coordinate care within the hospice interdisciplinary team.
• Communicate with attending and hospice physicians involved in care.
• Demonstrate accountability, flexibility, respect, and integrity.
• Comply with legal requirements, company standards, policies, and procedures.
• Timely submit all required paperwork.

A day in the life:

• RN will administer skilled nursing for all ages in their place of residence.
• RN will conduct patient admissions.

Qualifications:

• Current Registered Nurse license in the state which you will be working in.
• One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred.
• Must have a current Driver's license and dependable transportation.
