Job summary

Trinity Hospice in Texas is seeking a dedicated RN Case Manager to assess patients, develop care plans, and coordinate services to ensure comfort and quality of life for patients and families.

You will work closely with physicians, social workers, chaplains, and hospice aides to deliver compassionate, high-quality care while maintaining thorough documentation and regulatory compliance.

Qualifications

• Current Registered Nurse license in the state where you will work.
• One year experience as an RN Case Manager or in a similar hospice role preferred.
• Must have a valid driver's license and dependable transportation.

Responsibilities

• Perform comprehensive nursing assessments to identify physical, psychosocial, and emotional needs.
• Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort and quality of life.
• Provide skilled nursing interventions, including pain and symptom management and medication administration.
• Utilize critical thinking to recognize changes in condition and intervene appropriately.
• Serve as the primary RN case manager, coordinating care across the interdisciplinary team.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, and hospice aides.
• Facilitate communication among team members to support timely care planning.
• Maintain accurate nursing documentation in accordance with regulatory standards (Wellsky/Consolo).
• Participate in interdisciplinary team meetings and ongoing care planning.

A Day in the Life:

Qualifications:

• Current Registered Nurse license in the state which you will be working in.
• One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred.
• Must have a current Driver's license and dependable transportation.
