Job summary

Trinity Hospice is seeking an experienced RN to serve as a case manager, coordinating patient-centered hospice care. The role emphasizes assessments, care planning, symptom management, and collaboration with an interdisciplinary team to ensure high-quality outcomes.

This PRN-like position requires an active RN license in the state of work, with eligible experience in hospice settings and reliable transportation to meet patient needs.

Qualifications

• Active RN license in the state where you work.
• Minimum 1 year of RN Case Manager or hospice case management experience.
• Valid driver''s license and reliable transportation.

Responsibilities

• Perform comprehensive nursing assessments of patients and families to identify physical, psychosocial, and emotional needs.
• Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort, symptom control, and quality of life.
• Provide skilled nursing interventions, including pain and symptom management, wound care, and ongoing clinical monitoring.
• Coordinate care across the interdisciplinary team as the primary RN case manager.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, and hospice aides for seamless care delivery.
• Maintain accurate and thorough nursing documentation in accordance with regulatory standards.

A Day in the Life:

• Collaborate closely with physicians, nurse practitioners, social workers, chaplains, and hospice aides to ensure seamless care delivery
• Facilitate communication among team members to support timely and effective care planning
• Maintain accurate, timely, and thorough nursing documentation in accordance with regulatory and organizational standards (Wellsky/Consolo)
• Participate actively in interdisciplinary team (IDT) meetings, care conferences, and ongoing care planning

Qualifications:

• Current Registered Nurse license in the state which you will be working in.
• One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred.
• Must have a current Driver's license and dependable transportation.
