Job summary

Trinity Hospice 1 LLC in Charlottesville, VA is seeking an experienced RN Case Manager to provide patient-centered hospice care across the interdisciplinary team. You will assess needs, manage symptoms, and coordinate access to comfort-focused care.

The role involves collaboration with physicians, social workers, chaplains, and aides, maintaining thorough documentation and supporting ongoing care planning.

Qualifications

• Current Registered Nurse license in the state where you will work.
• 1+ year experience as an RN Case Manager or similar role in a hospice agency preferred.
• Must have a valid driver's license and dependable transportation.

Responsibilities

• Perform comprehensive nursing assessments to identify physical, psychosocial, and emotional needs.
• Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort and quality of life.
• Provide skilled nursing interventions including pain and symptom management, wound care, and clinical monitoring.
• Serve as primary RN case manager coordinating care across the interdisciplinary team.
• Collaborate with physicians, NPs, social workers, chaplains, and hospice aides to ensure seamless care.

A Day in the Life:

• Perform comprehensive nursing assessments of patients and families to identify physical, psychosocial, and emotional needs

• Utilize critical thinking and clinical judgment to recognize changes in condition and intervene appropriately
• Serve as the primary RN case manager, coordinating care across the interdisciplinary team
• Collaborate closely with physicians, nurse practitioners, social workers, chaplains, and hospice aides to ensure seamless care delivery
• Facilitate communication among team members to support timely and effective care planning
• Maintain accurate, timely, and thorough nursing documentation in accordance with regulatory and organizational standards (Wellsky/Consolo)
• Participate actively in interdisciplinary team (IDT) meetings, care conferences, and ongoing care planning

Qualifications:

• Current Registered Nurse license in the state which you will be working in.
• One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred.
• Must have a current Driver's license and dependable transportation.
