Job summary

Pioneer Valley Hospice & Palliative Care in Greenfield, MA is seeking a Referral and Admission Coordinator for a full-time, 40-hour workweek. You will manage all referrals, verify eligibility, and coordinate with hospitals, physicians, and families to ensure timely admission to PVHPC services.

The role requires excellent communication, meticulous data entry, and the ability to balance multiple tasks in a patient-centered, compliant environment.

Qualifications

• 1–2 years of call center experience in a clinical/medical office.
• Hospice and customer service experience is preferred.
• Healthcare-related degree or certificate required or preferred.

Responsibilities

• Manage all incoming referrals and ensure compliance with policies.
• Coordinate with physicians, discharge planners, and facilities for admission.
• Input and verify patient information in the EMR and track referral status.
• Inform referral sources of admission decisions and patient eligibility.
• Maintain relationships with PVHPC and affiliate referral sources.
