Gentiva Hospice is seeking a Hospice Care Consultant to champion our mission while developing strong referral relationships with hospitals, home health agencies, SNFs, clinics, and physician offices. You will educate healthcare professionals about hospice options and drive timely patient referrals with a compassionate, strategy-driven approach.

Qualifications

• Bachelor's degree or equivalent experience in business, marketing, or communications.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs or home health professionals.
• Prior hospice, palliative care, or home health experience is a plus.
• Excellent communication and time-management skills.
• Proficient in MS Office and CRM platforms.
• Sensitive to needs of terminally ill patients and families.
• Proven track record of meeting admissions and census targets.

Responsibilities

• Educate, inform, and inspire confidence in our services for proper patient referrals.
• Identify new referral opportunities by calling on hospitals, SNFs, home health agencies, and clinics.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice benefits.
• Plan and execute strategic marketing initiatives to increase awareness and drive referrals.
• Represent Gentiva at community events, associations, and educational in-services.
• Serve as a resource for patients and families, guiding hospice options and early referrals.
