Job summary

Amedisys is seeking a hospice liaison in the Portland, ME area to educate health care professionals, patients, and families about home care services. This outside sales role focuses on developing referral sources and coordinating admissions with the team to ensure smooth referrals.

The ideal candidate has at least one year in healthcare business development, a valid driver's license, reliable transportation, and is comfortable traveling 50% of the time within the territory of Portland,

Qualifications

• Experience in healthcare-related business development/outreach.
• Bachelor’s degree preferred, but not required.
• Reliable transportation, valid driver's license, and liability insurance.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team and handles referrals in the admission process.
• Conducts on-site visits to assess patient needs and identify barriers to care.
• Keeps accurate records of patient interactions and referrals.
• Achieves admissions, product, and payor-mix goals.
