AMEDISYS HOME HEALTH is seeking a hospice liaison in the Green Bay/Appleton area. The role emphasizes outside sales and developing healthcare referral networks to drive home health and hospice admissions.

The ideal candidate has at least one year of healthcare business development experience; a bachelor's degree is preferred but not required. Travel is 50%+ within the territory, with competitive base pay and bonus potential.

Responsabilidades

• Educate health care professionals, patients, and families about home care services.
• Develop and maintain relationships with referral sources to generate leads.
• Coordinate patient admissions and obtain documentation for referral to admission.
• Conduct on-site visits to assess patient needs and barriers to care.
• Maintain accurate records of patient interactions and referrals.
• Meet admissions, product, and payer-mix goals; achieve short/long-term targets.
• Log daily sales calls in CRM with notes; keep territory sources updated.
• Attend community events to promote Amedisys and respond to complaints.
