Job summary

Bon Secours Home Care and Hospice by Compassus is seeking a Hospice Clinical Liaison who embodies compassionate care and upholds theCompassus pillars. The role focuses on identifying patients whose needs align with hospice services and coordinating admissions from hospitals and long-term care facilities.

The ideal candidate is a registered nurse with 2–3 years of clinical experience, preferably with hospital or long-term care exposure, and some healthcare marketing knowledge.

Qualifications

• RN License is required; Bachelor’s degree in Nursing highly preferred.
• Two to three years of related experience.
• Hospital and/or long-term care clinical experience highly preferred.
• Experience in healthcare marketing preferred.

Responsibilities

• Attends and participates in staff meetings, educational programs, and community events.
• Explains hospice services and benefits to patients, family members, and significant others in a manner that facilitates an accurate and positive view of hospice services.
• Coordinates and facilitates the admission process from hospital and long-term care facilities.
• Assesses referred patient’s clinical needs and confers with physician to establish eligibility.
• Oversees discharge planning from hospital to home with required services and coordination.
