Job summary

The Care Team Hospice is seeking a compassionate Physical Therapist (PT) to support hospice patients across Akron, Cuyahoga Falls, OH, and surrounding communities on a PRN, paid-per-visit basis. This role emphasizes comfort, safety, and dignity for patients and families in their homes and facilities.

You will evaluate mobility, design interventions aligned with the hospice plan, and educate caregivers while collaborating with an interdisciplinary team.

Qualifications

• Active and unrestricted Physical Therapist license in Ohio.
• Graduate of an accredited Physical Therapy program.
• Hospice/home health/geriatric experience preferred.
• Strong clinical assessment, communication, and patient education skills.

Responsibilities

• Complete individualized physical therapy evaluations focused on mobility, strength, balance, safety, functional abilities, and patient comfort.
• Develop and provide appropriate therapy interventions aligned with each patient's hospice plan of care and personal goals.
• Recommend assistive devices, safe transfer techniques, positioning strategies, and environmental modifications to improve comfort and safety.
• Educate patients, families, and caregivers on mobility, fall prevention, energy conservation, positioning, and safe daily activities.
• Collaborate with the interdisciplinary hospice team and complete accurate, timely clinical documentation.
