Job summary

The Care Team Hospice is seeking a compassionate Physical Therapist to support hospice patients in Akron, Cuyahoga Falls, OH, and surrounding communities on a PRN, paid-per-visit basis.

You will conduct individual PT evaluations and provide interventions in patients' homes and facilities, working independently while collaborating with an experienced hospice team to improve comfort, safety, and quality of life.

Qualifications

• Active Ohio PT license required.
• Graduate of an accredited Physical Therapy program.
• Previous PT experience; hospice/home health or geriatric experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform individualized PT evaluations focusing on mobility, strength, balance, safety, and comfort.
• Develop and implement interventions aligned with hospice plan of care and patient goals.
• Recommend assistive devices, safe transfer techniques, positioning, and environmental modifications.
• Educate patients, families, and caregivers on mobility, fall prevention, energy conservation, and safe activities.
• Collaborate with the interdisciplinary hospice team and complete accurate clinical documentation.
