Job summary

The Care Team Hospice is seeking a compassionate Physical Therapist (PT) to support hospice patients in the Greater Youngstown area on a PRN, paid-per-visit basis. This role offers flexible scheduling, independence in the field, and meaningful patient care in home and facility settings.

You will perform PT evaluations focused on mobility, balance and safety, develop interventions aligned with the patient plan of care, educate families, and collaborate with an interdisciplinary hospice team.

Qualifications

• Active and unrestricted Physical Therapist license in Ohio.
• Graduate of an accredited Physical Therapy program.
• Hospice/home health or geriatric experience preferred.
• Strong clinical communication and patient education skills.

Responsibilities

• Complete individualized PT evaluations focusing on mobility, strength, balance and safety.
• Develop and provide interventions aligned with the hospice plan of care.
• Recommend assistive devices, safe transfer techniques and positioning strategies.
• Educate patients, families and caregivers on mobility and daily activity safety.
• Collaborate with the interdisciplinary hospice team and document promptly.
