Job summary

The Care Team Hospice is seeking a compassionate and experienced Physical Therapist (PT) to support hospice patients in the Greater Youngstown Area on a PRN, paid-per-visit basis.

Qualifications

• Active PT license in Ohio is required.
• Graduate of an accredited Physical Therapy program.
• Hospice/home health/geriatric experience preferred.

Responsibilities

• Perform individualized physical therapy evaluations focused on mobility, strength, balance, safety, and comfort.
• Develop interventions aligned with the hospice plan of care and patient goals.
• Educate patients and caregivers on mobility, fall prevention, and safe daily activities.
• Collaborate with the interdisciplinary hospice team and document care promptly.
