Job summary

Gentiva is seeking a Hospice Physician to ensure compassionate, goal-directed care for hospice patients, collaborating with interdisciplinary teams, certifying eligibility, and guiding quality improvement.

This safety-sensitive role requires a medical license, ABMS/AOA-BOS board certification, and ability to travel 15–25% to support program needs. You will document thoroughly and participate in policy development and community partnerships.

Qualifications

• MD or DO with hospital or hospice certification and license.
• ABMS/AOA-BOS board certification and hospice/PM speciality preferred.
• Active, unrestricted medical license in practicing state.
• NPI and DEA registrations for controlled substances.
• Medicare/Medicaid enrollment eligibility where required.
• Willingness to travel 15–25% for program needs.
• CPR and TB clearance required.

Responsibilities

• Certify and recertify hospice eligibility, ensuring Medicare compliance.
• Conduct and supervise home visits and complex case consultations.
• Provide 24/7/365 on-call physician coverage coordination.
• Maintain timely medical documentation for regulatory reviews.
• Participate in IDG meetings to coordinate care and meds.
• Monitor pharmacy use and appropriate prescribing/administration.
• Collaborate with leadership on policy, compliance, and QAPI.
• Serve as community liaison to expand hospice partnerships.
• Support staff, physicians, and community education programs.
