Job summary

Gentiva Health Services, Inc. is seeking a Hospice Physician to deliver compassionate, goal-directed care to hospice patients. You will be responsible for certifying eligibility, conducting patient visits, and collaborating with interdisciplinary teams.

Qualifications

• Doctorate degree in allopathic or osteopathic medicine from an accredited medical school.
• Board certification in a specialty recognized by ABMS or AOA-BOS, preferably primary care.
• Current and unrestricted medical license in the practicing state.
• Hospice and Palliative Medicine board certification preferred.
• Ability to travel 15-25% based on program needs.

Responsibilities

• Certify and recertify hospice eligibility, ensuring timely compliance.
• Conduct and supervise patient visits, including complex home consultations.
• Participate actively in interdisciplinary group meetings for coordinated care.
• Support quality assessment and performance improvement initiatives.
• Serve as a community liaison, enhancing partnerships with healthcare providers.
