Job summary

You will conduct comprehensive assessments, manage symptoms, oversee admissions and certifications, lead goals of care discussions, and collaborate with a multidisciplinary team to ensure seamless patient-centered care. Qualifications

• MD or DO from an accredited medical school.
• Current CPR certification.
• Current license to practice in the relevant state.
• Membership in good standing on the medical staff of at least one contracted hospital.
• Valid driver’s license and agency-required liability insurance for home visits.

Responsibilities

• Conduct comprehensive, face-to-face assessments with patients and manage symptoms.
• Oversee admissions, discharges, certifications, and recertifications and medication management.
• Lead goals of care discussions with patients and families.
• Maintain accurate medical records and ensure regulatory compliance.
• Provide on-call coverage for medical emergencies and collaborate with attending physicians.
