Job summary

Hospice Savannah seeks a Physical Therapist to provide comprehensive PT services to PACE participants, including assessment, care planning, intervention, education, and ongoing monitoring to support mobility and safety.

The role collaborates with the Interdisciplinary Team to ensure needs are reflected in individualized care plans and contributes to care coordination, documentation, and caregiver training within the PACE model.

Qualifications

• Master’s degree in Physical Therapy is required.
• Valid driver’s license and reliable transportation are required.
• One year of experience with frail or elderly population, or willingness to receive training.
• Must not have convictions related to Medicaid/Medicare or health programs, or exclusions from participating in these programs.

Responsibilities

• Provide comprehensive physical therapy services to PACE participants, including assessment and care planning.
• Deliver interventions, education, and ongoing monitoring to support functional mobility and safety.
• Collaborate with Interdisciplinary Team to incorporate needs into individualized care plans.
• Contribute to care coordination, documentation, quality initiatives, and caregiver training.
