Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to deliver skilled care in the home, focusing on mobility, independence, and daily living tasks. You’ll apply modalities, educate families, and participate in specialty programs under physician orders and supervision.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Treat patients with physical impairments using physical modalities.
• Help patients improve mobility, independence and daily living skills.
• Provide specialty programs: Balance and Fall Prevention, Spine Safety, Hip/Knee and Shoulder programs.
• Administer skilled care under physician orders and plan of care.
• Educate family and health team members; work under Branch Director supervision.
