Job summary

Sobe Innovative Rehab in Pooler, GA is seeking a licensed Physical Therapist (DPT, PT, RPT) to provide hospice therapy in a home setting on a PRN basis. You will evaluate residents, implement individualized plans, and collaborate with physicians and families to support independence and quality of life.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license
• New graduates and experienced therapists are encouraged to apply

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR
