Job summary

Confluence Health is seeking a Pharmacist to deliver clinical and operational pharmacy services within our hospice and outpatient setting. The role involves therapeutic assessment, intervention planning, and collaboration with an interdisciplinary care team to improve patient outcomes.

The position reports to the Outpatient/Hospice Pharmacy Manager and requires active licensure in Washington, CPR certification, and potential Hospice certification within one year.

Qualifications

• Pharmacist degree (B.S. or PharmD) and WA license.
• CPR certification within 90 days of hire.
• Hospice and Palliative Care certification within 1 year.

Responsibilities

• Provide clinical and operational pharmacy services.
• Perform therapeutic assessment and care plans for patients.
• Supervise pharmacy technicians and assistants.
• Coordinate with Hospice physicians and nurses.
