Ohio’s Hospice in Dayton, OH is seeking a full-time Hospice Patient Care Specialist (PCS) for the day shift. This 40-hour-per-week role involves delivering compassionate care to patients in facilities and homes across Centerville and surrounding areas, working with an interdisciplinary team.

You will document services, drive during the day or night, and support families during difficult times. Qualifications include a high school diploma or GED; Ohio CNA preferred; valid driver’s license; and

Qualificações

• High school diploma or GED is required.
• Certified Nursing Assistant with the State of Ohio preferred.
• One year of experience preferred.
• Certification in Hospice & Palliative Medicine (CHPNA) preferred within two years of employment.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's license with Safe Driving Record.
• State minimum automobile insurance coverage.
