Job summary

You will direct patient care services, lead IDG meetings, manage scheduling, and mentor associates while ensuring adherence to Medicare/Medicaid and JCAHO/ACHC guidelines. A BSN is preferred and EMR/telehealth familiarity is valued.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting.
• Knowledge of hospice principles, Medicare/Medicaid, JCAHO, ACHC regulatory compliance.
• Knowledge and compassion for terminally ill patients and families.
• Ability to lead a diverse care team while adapting to emotional/clinical needs.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor and train associates to support development, performance, and compliance.
• Ensure care delivery meets federal, state, and local regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies at the location.
