Christus Hospice St. Frances Cabrini, a part of LHC Group, is seeking a Patient Care Manager - Hospice to support Executive leadership in delivering safe, compliant home hospice care.

This role focuses on coordinating patient care across the team and ensuring regulatory standards are met. You will supervise staff, participate in investigations of issues, provide in-services, and work in diverse home settings.

Formación

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• 4+ years full-time experience as an RN or equivalent.
• 1+ years of clinical experience.
• Able to work independently.
• Good communication, writing, and organizational skills.

Responsabilidades

• Responsible for compliance with hospice policies and regulatory requirements governing the practice of the assigned team.
• Actively promotes and directs assigned team regarding quality of care and safety of patients and staff.
• Engages in thorough problem resolution and complaint investigation.
• Provides in-services to staff to promote health and safety, address regulatory issues and agency policy.
• Ability to function in any home situation regardless of client background.
