Job summary

Cabrillo Hospice seeks an experienced Hospice Account Executive to manage referral partner relationships across LA County/North OC, ensuring seamless patient transitions into hospice care. The role emphasizes education, care coordination, and cross-functional collaboration rather than sales quotas.

Ideal candidates will have 2+ years in healthcare account management, strong communication skills, and a valid driver’s license to support travel across the region.

Qualifications

• Experience in hospice, home health, or healthcare account management strongly preferred.
• Minimum of two (2) years of experience working with physicians, hospitals, insurance providers, skilled nursing facilities, or other healthcare partners.
• Strong communication, presentation, and relationship-building skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop, manage, and maintain strong relationships with assigned referral partners, including hospitals, physicians, SNFs, and other healthcare providers.
• Serve as the primary point of contact for referral sources, ensuring consistent communication and responsiveness.
• Collaborate with discharge planners, case managers, social workers, and clinical teams to ensure smooth patient transitions into hospice care.
• Monitor referral processes to ensure patient needs, expectations, and care plans are understood and met.
• Provide ongoing education to referral partners regarding hospice eligibility, services, benefits, and care coordination processes.
• Support referral sources by addressing questions, concerns, and operational challenges related to hospice services.
• Work with the interdisciplinary hospice team to align patient care goals and improve referral workflows.
• Participate in team meetings, care coordination discussions, and performance reviews.
• Identify opportunities to improve communication, service delivery, and partner satisfaction.
• Represent Cabrillo Hospice at community meetings, healthcare events, and professional gatherings.
