Job summary

Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to deliver advanced home-based palliative care and guide patients and families. You will work with the Palliative Medical Director, perform thorough assessments, and create individualized plans that improve quality of life.

Requires MSN, 2+ years as NP, and knowledge of palliative care, Medicare/Medicaid rules, and Part B billing. This is a safety-sensitive, full-time role with remote Wisconsin coverage.

Qualifications

• Master of Science in Nursing from an accredited program.
• Minimum two years of Nurse Practitioner experience.
• Family or Adult Medicine expertise preferred.
• Experience with Part B billing and autonomous practice.

Responsibilities

• Collaborate with the Palliative Medical Director and care team to deliver patient-centered palliative care.
• Perform in-depth patient assessments and develop individualized care plans.
• Manage and adjust treatment plans and prescribe medications as permitted by state law.
• Maintain timely clinical documentation and communicate with the care team.
• Participate in evening/weekend call rotation and facilitate referrals as needed.
