Job summary

Gentiva Hospice in Rhode Island seeks a Nurse Practitioner for a flexible PRN schedule covering days, evenings, weekdays and weekends. The role includes hospice eligibility evaluations, care planning, and collaboration with interdisciplinary teams to provide patient-centered, end-of-life care.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred; hospice/palliative care experience preferred.

Responsibilities

• Perform hospice and palliative NP duties, including F2F visits for eligibility and recertification.
• Collaborate with IDG/IDT to develop and manage patient care plans.
• Prescribe medications and manage symptoms within regulatory scope.
