Job summary

Gentiva Hospice is seeking a Nurse Practitioner to join our PRN team in Rhode Island. The role focuses on hospice eligibility assessments (F2F visits) and comprehensive palliative care, blending autonomy with collaboration across IDG/IDT teams.

You'll perform patient management visits, support goals of care, and coordinate with physicians, PCPs, and specialists to deliver compassionate, high-quality care within CMS guidelines. Prior hospice/palliative experience is preferred.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Complete all visit types for patients on services, including Medicare-required hospice Face-to-Face (F2F) visits within CMS established regulatory timeframes.
• Collaborate with Hospice and Palliative Medical Directors and interdisciplinary teams to deliver comprehensive, patient-centered care.
• Collaborate with PCP/Attending Provider and specialists to coordinate patient Plan of Care.
• Perform in-depth patient assessments across physical, emotional, functional, and psychosocial domains.
• Support individualized care plans in partnership with patients, families, caregivers, and IDG/IDT.
• Prescribe medications and treatments as permitted by state law and within Independent Practice or Collaborative Practice Agreement.
• Order and interpret laboratory and other diagnostic tests within scope.
• Facilitate goals-of-care discussions and advance care planning.
• Coordinate care across settings (home, SNFs, assisted living).
• Initiate referrals to supportive services and community resources.
