Job summary

PruittHealth Hospice is seeking a PRN Nurse Practitioner to provide hospice and palliative care services in patients’ homes or care facilities. The role emphasizes comprehensive adult/geriatric assessments, plan-of-care management, and rigorous documentation to ensure continuity of care in a compassionate, patient-centered environment.

Qualified candidates hold an active RN license and NP board certification (AANP/ANCC).

Qualifications

• Current, active, and unrestricted RN license in state of practice.
• Board-certified NP (AANP or ANCC) in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
• Commitment to caring for patients and partners
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
