Job summary

Saint Francis Hospice in Brevard County, Florida, seeks a dedicated Hospice and Palliative Physician to join our interdisciplinary care team. You will direct medical care, validate prognosis, and collaborate with primary physicians to ensure high-quality hospice services.

You will lead IDT meetings, supervise care plans, and provide both inpatient rounds and home visits. Florida licenses, board certification in HPM or HMDC preferred, and commitment to compassionate patient care are required.

Qualifications

• Graduate of an accredited medical school or school of osteopathy.
• Board Certification in Hospice and Palliative Medicine (HPM) or Hospice Medical Director Certification preferred.
• Active, unrestricted Florida medical license (Chapter 458 or 459) with hospital admitting privileges in the service area.
• Active Medical and DEA license.
• Strong understanding of hospice and palliative care concepts and patient-family communication.

Responsibilities

• Direct the medical component of patient care and validate prognosis.
• Lead the Interdisciplinary Group (IDG) meetings and oversee plans of care.
• Prescribe and oversee pharmacotherapeutics with care team.
• Conduct Face-to-Face visits to confirm hospice eligibility.
• Provide community home visits for acute medical management and follow-up.
• Complete documentation, billing, and death certificates within Florida requirements.
• Provide after-hours, weekend, and holiday on-call coverage.
• Support compliance with federal, state, and local regulations.
