Job summary

Saint Francis Hospice is seeking a dedicated Hospice and Palliative Physician to join our interdisciplinary care team serving patients and families in Brevard County, Florida. You will direct medical care, validate prognosis, and collaborate with patients’ primary physicians to ensure high-quality hospice services.

Qualifications

• Graduate of an accredited medical school or osteopathy school.
• Board Certification in Hospice and Palliative Medicine (HPM) or Hospice Medical Director Certification (HMDC) preferred.
• Active, unrestricted Florida medical license (Chapter 458 or 459) with hospital admitting privileges in the service area.
• Active Medical and DEA licenses.
• Strong understanding of hospice and palliative care.
• Comfort with EMR systems and MS Office.
• Emotional resilience and ability to navigate sensitive conversations.

Responsibilities

• Evaluate admissions and assess prognosis while considering comorbidities.
• Round on inpatient hospice patients and provide weekend coverage as needed.
• Lead IDG meetings and oversee plans of care.
• Prescribe and oversee pharmacotherapeutics with care team.
• Conduct Face-to-Face visits to confirm ongoing eligibility.
• Provide home visits for acute medical management and follow-up.
• Complete documentation, billing, and death certificates within Florida’s 72-hour requirement.
• Provide after-hours, weekend, and holiday on-call coverage.
• Participate in the QAPI program and quality initiatives.
• Support compliance with federal, state, and local regulations.
