Job summary

Pallatus Healthcare seeks a compassionate Hospice and Palliative Care Physician to lead our interdisciplinary care team and deliver dignified end-of-life care. You will assess patients, develop tailored plans, and coordinate with nurses, social workers, chaplains, and specialists to ensure comfort and quality of life.

Ideal candidates hold an MD or DO with board certification in IM/FM or related specialty, possess a valid license and DEA, and have 2–3 years of relevant clinical experience in

Qualifications

• MD or DO from an accredited institution.
• Board certification in Internal Medicine, Family Medicine, or related specialty.
• Valid state medical license and DEA registration.
• Minimum 2–3 years of clinical experience, preferably in hospice, palliative care, or related field.
• Strong communication and interpersonal skills with ability to discuss sensitive topics compassionately.
• Commitment to interdisciplinary team‑based care approach.
• Electronic health record (EHR) proficiency.
• Valid driver’s license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive medical assessments and develop personalized care plans for hospice patients.
• Manage complex pain and symptom control protocols with expertise in palliative medicine.
• Lead interdisciplinary team meetings and coordinate care with nurses, social workers, chaplains, and other specialists.
• Provide on‑call coverage and respond to urgent patient needs in home and facility settings.
• Communicate compassionately with patients and families about prognosis, care options, and end‑of‑life planning.
• Document patient encounters thoroughly and maintain compliance with regulatory standards.
• Mentor and educate team members, patients, and families on hospice and palliative care principles.
• Participate in quality improvement initiatives and community outreach programs.
