Job summary

Saint Francis Hospice is seeking a dedicated Hospice and Palliative Physician to join our interdisciplinary care team serving patients and families throughout Brevard County, Florida. The physician will direct the medical component of care, validate prognosis and eligibility, and partner with patients' primary physicians to ensure high-quality hospice services.

Qualifications

• Graduated from an accredited medical school or school of osteopathy.
• Board Certification in Hospice and Palliative Medicine (HPM) or Hospice Medical Director Certification (HMDC) preferred.
• Active, unrestricted Florida medical license (Chapter 458 or 459) with hospital admitting privileges.
• Active Medical and DEA license.
• Strong understanding of hospice and palliative care.

Responsibilities

• Direct the medical component of patient care within the IDT.
• Validate prognosis and eligibility for hospice care.
• Lead Interdisciplinary Group (IDG) meetings and oversee care plans.
• Prescribe and oversee pharmacotherapeutics in collaboration with case managers and physicians.
• Conduct Face-to-Face visits to confirm ongoing hospice eligibility.
• Provide community home visits for acute medical management and follow-up.
• Complete documentation, billing, and death certificates within Florida's 72-hour window.
• Provide after-hours, weekend, and holiday on-call coverage.
• Participate in the Quality Assessment and Performance Improvement (QAPI) program.
• Support compliance with federal, state, and local regulations.
