Job summary

Saint Francis Hospice in Brevard County, Florida, seeks a dedicated Hospice and Palliative Physician to join our interdisciplinary care team. You will direct the medical component of patient care and guide prognosis and eligibility in partnership with patients’ primary physicians.

You’ll provide clinical leadership, mentorship, and guidance to staff and volunteers, ensuring compassionate, dignified care during life’s final chapters for families across the county.

Qualifications

• Graduate of an accredited medical school or osteopathy.
• Board Certification in Hospice and Palliative Medicine or HMDC preferred.
• Active Florida medical license with hospital admitting privileges.
• Active Medical and DEA license.
• Strong understanding of hospice and palliative care.
• Comfort with EMR systems and MS Office.
• Emotional resilience and ability to navigate sensitive conversations.
• Reliable attendance and commitment.

Responsibilities

• Evaluate hospice admissions and assess eligibility.
• Round on inpatient patients and provide weekend coverage.
• Lead IDG meetings and oversee comprehensive care plans.
• Prescribe pharmacotherapy with case managers and providers.
• Conduct Face-to-Face visits to confirm eligibility.
• Provide community home visits for medical management and follow-up.
• Complete documentation and Florida 72-hour billing requirements.
• Provide after-hours, weekend, and holiday on-call coverage.
• Participate in the hospice QAPI program.
• Ensure compliance with federal, state and local regulations.
