Qualifications
1 year in medical-surgical, ICU, home health, hospice, SNF or ALF settings.
Bachelor’s degree in nursing.
RN license in WA or OR within 3 months; Compact license acceptable.
Current Basic Life Support (BLS).
Valid driver’s license for patient home visits.
Typing test required; reliable automobile for travel.
Responsibilities
Assess patient/family needs and consult with physicians and IDT members to plan care.
Coordinate hospice/palliative care across settings and ensure smooth transitions.
Instruct caregivers and facility staff in hospice care basics and documentation.
Participate in 24-hour on-call rotation and complete ongoing documentation.
