Qualifications

• One year of medical-surgical or ICU nursing in acute care, ambulatory, home health, hospice or palliative care.
• Two years of hospice experience (within last 5 years); LPN may substitute for one year.
• Bachelor's degree in nursing or commensurate knowledge/experience.
• Credentials from multiple states required; primary state at hire with others post hire.

Responsibilities

• Assess patient/family needs and consult with physicians and the care team.
• Collect data on physical, psychosocial and spiritual factors to plan care.
• Facilitate end-of-life discussions with patient and family.
• Manage a patient/caregiver caseload through nursing process and scheduling.
• Coordinate hospice care across settings for smooth transitions.
• Attend weekly case conferences and document care plans.
• Provide input into bereavement care and participate in 24-hour on-call duties.
