Responsibilities

Develop and manage case plans for palliative/oncology patients using interdisciplinary teams.
Assess patient and family needs, eligibility, and education for admissions.
Coordinate resources, authorize services, and monitor care quality.
Disposition planning and transition to home or facility.
Liaise between care teams, families, and external resources.
Attend case management rounds and document patient progress.
