Responsibilities

• Plan, develop, assess, and evaluate care provided to members.
• Collaborate with physicians and the multidisciplinary team to develop individualized care plans.
• Recommend alternative levels of care and ensure compliance with regulations.
• Assess high-risk patients for post-hospital care planning.
• Develop and coordinate discharge plans to meet patient needs.
• Review and monitor patient hospital stays for timely service delivery.
• Participate in Bed Huddles and implement recommendations.
• Coordinate continuity of care, including utilization management and discharge planning.
• Conduct daily clinical reviews for utilization/quality management.
• Act as a liaison between inpatient and referral facilities.
