Job summary
CorsoCare is seeking a Nurse Practitioner to perform face-to-face assessments of hospice patients across multiple settings, coordinating with physicians and care teams to determine eligibility and provide symptom management interventions. The NP will operate under supervision of the Hospice Physician or Medical Director.
The role requires active NP licensure in the practicing state and a current DEA license, with preference for candidates who have substantial nursing experience in oncology,
Qualifications
- Certified/licensed Nurse Practitioner in the practicing state.
- DEA License in the practicing state.
- In good standing to participate in Medicare/Medicaid programs.
- Minimum of 5 years nursing experience; hospice/oncology/geriatrics preferred.
Responsibilities
- Accountable for an assigned caseload of patients to a developed plan of care.
- Adheres to HIPAA and applicable privacy/security standards.
- Provides care in accordance with standards, guidelines, law, and regulations.
- Develops and updates a comprehensive patient-centered care plan with physician orders.
- Makes referrals to other services as indicated.
- Prepares and maintains clinical documentation per agency policies.
