Qualifications

• Current RN license in the state of Ohio.
• Bachelor's degree in Nursing (BSN) or related field.
• Minimum 2 years of experience in Hospice Palliative Care.
• Knowledge of current nursing standards, practices, and regulations.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Lead a team of Hospice Palliative Care RNs in providing compassionate and high-quality care to patients with life-limiting illnesses.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans that focus on symptom management, pain control, and improvement of quality of life.
• Provide expert nursing assessment, intervention, and education to patients and their families.
• Manage care transitions and ensure continuity of care across settings.
• Conduct initial and ongoing assessments, identifying and addressing physical, emotional, and spiritual needs.
