Qualifications

• Graduate of an accredited school of nursing.
• Current, unrestricted RN license in the State of Ohio.
• Minimum of 2 years of nursing experience in home hospice or skilled home care; hospice experience preferred.
• Previous clinical leadership or supervisory experience preferred.
• Knowledge of hospice clinical operations, Medicare requirements, state regulations, and ACHC standards.

Responsibilities

• Coordinate and oversee direct and indirect patient care services provided by clinical personnel.
• Provide supervision, guidance, and support to clinical staff to promote quality care and compliance with standards.
• Assist the Director of Clinical Services with clinical operations, staff development, and patient care oversight.
• Conduct medical record reviews and chart audits to ensure accurate documentation and quality care.
• Collect data and participate in Quality Assurance and Performance Improvement (QAPI) activities.
• Review intake screening information to determine patient needs before assigning staff.
• Assist with onboarding and orientation of new clinical employees.
• Provide in-service education, coaching, and ongoing support to clinical team members.
• Conduct home visits and provide direct patient care as needed.
• Monitor medical supply needs and ensure supplies for patient care.
• Support compliance with Medicare requirements, Ohio regulations, and ACHC standards.
• Participate in performance evaluations and provide feedback and training to staff.
