Amedisys Hospice is seeking a Liaison in the Birmingham area to educate clinicians, patients and families about home care services and to expand referral sources. This role requires building relationships with hospitals, nursing facilities and physician offices, and coordinating admissions with the care team.

The ideal candidate has outside sales or business development experience in healthcare, and is willing to travel 50% or more within the territory.

Qualifications

• Experience in healthcare business development or outreach.
• Bachelor's degree preferred but not required.
• Reliable transportation and valid driver's license.

Responsabilités

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team and completes referral to admission processes.
• Conducts on-site visits to assess patient needs and barriers to care.
• Maintains accurate records of patient interactions and referrals; tracks progress in CRM.
