Job summary

Amedisys Hospice Liaison in Pembroke, NC, is a field-based sales role that focuses on connecting healthcare providers with home health and hospice services. You will develop referrals, conduct on-site visits, and coordinate admissions with the care team.

The role travels 50%+ within Cumberland County, offering a base salary between $75,000 and $85,000 plus monthly incentives and a comprehensive benefits package including medical plans, PTO, and a 401(k).

Qualifications

• At least 1 year of experience in healthcare-related business development/outreach.
• Bachelor's degree preferred, but not required.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team and completes referral-to-admission process.
• Conducts on-site visits to assess patient needs and barriers to care.
• Keeps accurate records of patient interactions and referrals in CRM.
• Meets admissions, product, and payor-mix goals.
• Documents daily sales calls in CRM with pre/post call notes.
• Attends community events to promote Amedisys.
