Job summary

Affinity Hospice Management LLC is seeking an On-Call Hospice RN to respond to after-hours patient and family needs, perform urgent assessments, and provide skilled nursing care. The role requires an active Virginia RN license and hospice/homecare experience is preferred. You will work with the Hospice Medical Director and IDG to coordinate care, document thoroughly, and participate in after-hours admissions when needed. A $500 sign-on bonus is offered.

Qualifications

• Virginia RN license required.
• Graduate of an accredited school of nursing.
• Hospice/homecare experience preferred.

Responsibilities

• Respond to hospice patient census and family needs during after-hours shifts.
• Provide urgent clinical and psychosocial interventions and pain/symptom management.
• Collaborate with physicians and IDG to ensure continuity of care.
• Document care thoroughly and timely in medical records.
• Coordinate with vendors for DME, supplies and medications after hours.
