Job summary

Concho Hearts Hospice is seeking an experienced PRN On-Call Registered Nurse to provide after-hours care across Corpus Christi and surrounding communities. You will assess patients, relieve symptoms, and coordinate with physicians and the interdisciplinary team.

Responsibilities include responding to after-hours calls, performing visits as needed, managing pain, and documenting care. Texas RN license and hospice experience are required; travel after hours is expected.

Qualifications

• Active Texas RN license or NLC.
• Hospice on-call experience required.
• Experience with pain management and end-of-life care.
• Strong clinical assessment and decision-making skills.
• Able to travel to patient homes after hours.

We're looking for a compassionate, dependable, and experienced PRN On-Call Registered Nurse (RN) to provide after-hours coverage throughout Corpus Christi, TX, and surrounding communities.

Whether it's managing a change in condition, relieving symptoms, or reassuring a family during a difficult moment, our on-call nurses play an essential role in ensuring patients receive the care and support they deserve.

What You'll Do:

• Provide after-hours nursing coverage during nights, weekends, and holidays.
• Respond promptly to patient and family calls requiring nursing assessment or intervention.
• Complete in-person nursing visits when clinically indicated.
• Assess changes in patient condition and provide appropriate nursing interventions.
• Manage pain, symptoms, and other urgent patient care needs.
• Communicate with physicians and obtain necessary orders.
• Provide education, reassurance, and emotional support to patients and families.
• Coordinate with the interdisciplinary team to ensure continuity of care.
• Respond to patient falls, changes in condition, and other urgent clinical concerns.
• Assist with end-of-life care and death visits as needed.
• Complete accurate and timely documentation of all patient encounters, assessments, and interventions.
• Maintain communication with the on-call triage team and branch leadership.
• Provide thorough handoff communication to the daytime clinical team regarding after-hours patient needs.
• Follow hospice regulations, clinical standards, and company policies.

What We're Looking For:

• Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
• Previous hospice nursing experience required.
• Experience with pain management, symptom control, and end-of-life care.
• Strong clinical assessment and critical-thinking skills.
• Ability to independently assess patients and make appropriate clinical decisions within the RN scope of practice.
• Excellent communication and documentation skills.
• Compassionate, dependable, and responsive to patient and family needs.
• Ability to remain calm and professional during urgent or emotionally challenging situations.
• Comfortable traveling to patient homes and healthcare facilities after hours.
• Familiarity with electronic medical records (EMR).
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to provide coverage during nights, weekends, and holidays.

We're especially interested in nurses with previous hospice on-call experience who understand the importance of timely assessments, compassionate communication, and dependable follow-through.
