Job summary

Three Oaks Hospice is seeking a PRN On-Call Registered Nurse (RN) to join our team. You will administer skilled nursing care to patients in their homes and coordinate with the interdisciplinary team, patient, family, and referring agency to ensure seamless care.

The ideal candidate holds a current RN license, CPR certification, and at least two years of clinical nursing experience, preferably in hospice or home health, with strong communication skills for patient and family interactions.

Qualifications

• Graduate of an accredited Associate or Bachelor’s degree in Nursing.
• Current state RN license and a valid driver’s license.
• Two years’ RN experience in a clinical setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills to support patients and families.
• Ability to cope with family emotional stress and diverse lifestyles.

We are looking for a passionate PRN On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
