Job summary

Three Oaks Hospice, Inc. is seeking a compassionate Weekend PRN On-Call Registered Nurse to join our hospice team. You will provide skilled nursing care in patients' homes and coordinate with the interdisciplinary team while supporting families.

The ideal candidate holds an accredited RN license, CPR certification, and demonstrates strong communication and computer skills. Weekend on-call duties offer meaningful patient impact within a supportive, patient-centered network.

Qualifications

• Graduate of an accredited Associate or Bachelor's degree in Nursing.
• Current state RN license and valid driver's license.
• Two years RN experience; hospice or home health preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Ability to cope with family emotional stress.

Responsibilities

• Administer skilled nursing care for patients in their residence.
• Coordinate care with interdisciplinary team, patient, family, and referring agency.
• Provide on-call weekend nursing coverage as needed.

We are looking for a passionate On Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference by listening, caring and serving.

POSITION SUMMARY

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; and assumes the responsibility for coordination of care.

QUALIFICATIONS

• Graduate of an accredited Associate or bachelors degree in Nursing
• Current state license as an RN and current state drivers license
• Two years experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice ability to cope with family emotional stress and tolerance of individual lifestyles
