Job summary

Three Oaks Hospice, Inc. is seeking a PRN On-Call Registered Nurse (RN) to provide skilled nursing services in patients' homes within Clarke County and surrounding areas.

The role emphasizes coordinating with an interdisciplinary team, patients, and families to ensure continuity of care. The ideal candidate holds an RN license, a nursing degree, CPR certification, and prior hospice or home health experience.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice and ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their home and coordinate care with the interdisciplinary team.
• Coordinate care with patient/family and referring agency; update the plan of care as needed.
• Provide compassionate support to patients and families and assist in care planning and delivery.
